Job summary

CareMed is seeking a Nurse Practitioner to provide primary and preventive care for residents in long-term care facilities. You will manage acute changes in condition and participate in advanced care planning and chronic disease management.

Responsibilities include prescribing medications per OH law, ordering tests, and coordinating with nursing home staff and primary care physicians to ensure high-quality, interdisciplinary patient care.

Qualifications

• Must hold current, unrestricted NP license in Ohio.
• Board certification and DEA licensure required.
• Experience in long-term care and geriatric populations preferred.
• Proficient with EMR and coordinating with a care team.

Responsibilities

• Provide primary and preventive care for residents in skilled nursing facilities.
• Manage residents with acute changes in condition.
• Engage in advanced care planning and chronic disease management.
• Prescribe medications, including controlled substances per OH law.
• Order, review, and follow up on treatments and tests.
• Communicate changes in condition with nursing home staff.
• Collaborate with primary care physicians to coordinate care.
