Job summary

CareMed Health Solutions is seeking a Nurse Practitioner to provide primary and preventive care for residents in long-term care facilities in New Hampshire. The role involves managing acute changes, planning for advanced care and coordinating with an interdisciplinary team.

The ideal candidate has NH NP licensure, board certification and DEA licensure, with experience in geriatric settings and familiarity with EMR systems.

Qualifications

• Current, unrestricted NP license in New Hampshire.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team.
• Psych experience a plus

Responsibilities

• Provide primary and preventative care for long term care patients in skilled nursing facilities.
• Manage residents with acute changes in condition.
• Engage in advanced care planning and chronic care management.
• Prescribe medications in accordance with NH law, including controlled substances.
• Order, review and follow up with treatments and diagnostic testing.
• Communicate with nursing home staff about changes in condition.
• Collaborate with primary care physician as part of a care team.
