Job summary

PACE Southeast Michgan seeks a Nurse Practitioner to provide primary care for an assigned participant panel, under supervision of the center PIC. Responsibilities include chronic disease management, quarterly assessments, and care coordination across the interdisciplinary team.

The NP will perform prescribed diagnostics, collaborate with physicians, and support end-of-life discussions within the PACE framework. Clinic, home, and telemedicine locations are utilized.

Qualifications

• Three to five years of progressively more responsible experience as an RN in a clinical setting with geriatrics expertise.
• One year of experience with a frail or elderly population.
• Maintain current Michigan NP License and CME requirements.
• Comply with TB screenings, infection control, BLS and in-service requirements.

Responsibilities

• Provide chronic disease management and quarterly assessments.
• Coordinate with Physician-In-Charge and clinical team on treatment plans.
