Job summary

Senior Community Care of Michigan PACE, part of Volunteers of America National Services, seeks a Full-Time Nurse Practitioner to join an on-site care team in Lansing, MI. You will provide primary care, perform initial assessments, and develop comprehensive care plans with the interdisciplinary team to support frail elders.

The position offers a Monday–Friday schedule (8:00a–4:30p) with rotating on-call and occasional local travel to participants’ homes.

Qualifications

• Licensed as Registered Nurse (R.N.) in the state of employment.
• Graduate of a school of professional nursing required.
• Certified as a nurse practitioner by a national certifying organization recognized by the State Board of Nursing.
• A minimum of one year’s experience in working with the frail and elderly population required.

Responsibilities

• Provide primary care to PACE participants.
• Perform physical assessments of new enrollees and semi-annual reassessments.
• Develop and implement care plans with the Interdisciplinary Team (IDT).
• Educate participants and caregivers on health management.
• Coordinate with PCPs and Medical Directors of VOANS facilities.
