Job summary
Volunteers of America National Services (VOANS) Senior Community Care of Michigan - PACE is seeking a Full-Time Nurse Practitioner to provide primary care to frail elder participants onsite. The NP will perform assessments, reassessments, and develop comprehensive care plans under the Medical Director and Interdisciplinary Team (IDT).
The role emphasizes elder care, collaboration with physicians and directors, and on-site schedule with occasional travel for home visits.
Qualifications
- RN license in the state of employment is required.
- Graduate of a school of professional nursing.
- Certified as a nurse practitioner by a national certifying organization recognized by the State Board of Nursing.
- Minimum of one year experience with frail and elderly population.
Responsibilities
- Provide primary care to PACE enrollees under Medical Director supervision.
- Conduct physical assessments of new enrollees and semi-annual reassessments.
- Develop and implement plans of care for participants.
- Educate participants and caregivers; coordinate care with the IDT.
