Job summary

Humboldt Senior Resource Center in Eureka, CA is seeking a Nurse Practitioner to provide professional medical services to Redwood Coast PACE participants. The role requires leadership in primary care, collaboration with an Interdisciplinary Team, and care delivered in various settings including participant residences and clinics.

The NP will assess, plan, and monitor care quarterly or as conditions change, coordinate with family, and refer to specialists as needed.

Qualifications

• Graduate of an accredited nurse practitioner program.
• Five (5) years of primary care experience with at least three (3) years of experience with a frail or elderly population.
• Experience working as part of an Interdisciplinary Team preferred.

Responsibilities

• Provides professional medical services to Redwood Coast PACE participants.
• Conducts initial assessments and quarterly reviews with the Interdisciplinary Team.
• Develops and monitors a Plan of Care and coordinates with family and care providers.
• Provides primary care visits in residences and hospital admissions as needed.
• Orders and interprets diagnostic tests and prescribes medications as appropriate.
• Participates in team meetings and cares for participants in collaboration with RCPACE staff.
