Job summary

Redwood Coast PACE, a Program of All-inclusive Care for the Elderly, is seeking a full-time Registered Nurse (RN) in the Eureka region. The RN will provide comprehensive nursing care, coordinate plans of care with the Interdisciplinary Team, and deliver skilled nursing services in clinic and home settings to support frail older adults at home and in the Day Center.

The ideal candidate has a CA RN license, BLS certification, and at least one year working with elderly populations.

Qualifications

Must be a graduate of an accredited school of professional nursing.
California RN license in good standing; BLS certification required.
One year of experience with frail or elderly population preferred; hospital experience valued.
Strong communication and collaboration with interdisciplinary team.

Responsibilities

Conduct initial assessments and reassessments; develop Plans of Care with the IDT.
Provide direct skilled nursing in clinic and home settings, including wound care and IV therapy.
Coordinate medication management and arrange home delivery as needed.
Educate participants and families on disease management and self-care.
Participate in IDT meetings and quality improvement initiatives.
Maintain accurate documentation in medical records.
