Job summary

AllCare.ai seeks Primary Care NPs to provide on-site care to Assisted Living residents in Eureka and surrounding Humboldt County. The role involves delivering in-person care three days per week with clustered routes and occasional telehealth for flexibility.

Qualified candidates hold CA NP and RN licenses with NPI/DEA/BLS credentials preferred; geriatrics or facility-based experience and EMR proficiency are desirable. Compensation is structured around experience and patient volume.

Qualifications

• Active CA NP license and RN license required.
• Active furnishing license; NPI, DEA, and BLS credentials preferred.
• Geriatrics or facility-based experience preferred; EMR proficiency required.

Responsibilities

• Perform routine and urgent assessments and chronic disease management.
• Conduct post-discharge follow-ups, med reconciliation, labs and diagnostics.
• Coordinate with facility staff, specialists, families/POAs; ensure same-day documentation and CMS compliance.
