Job summary

AllCare.ai is seeking a Primary Care NP to deliver on-site care to Assisted Living residents in Eureka and the surrounding Humboldt County area. You’ll perform routine and urgent assessments, manage chronic diseases, and coordinate with facility staff.

This part-time role requires active CA NP and RN licenses, a furnishing license, NPI, DEA, and BLS. Clinicians with geriatrics or facility-based experience and EMR proficiency are preferred. W-2 employment with mileage reimbursement.

Qualifications

• Geriatrics or facility-based experience preferred; EMR proficiency.

Responsibilities

• Routine & urgent assessments; chronic disease management
• Post-discharge follow-ups, med reconciliation, labs/diagnostics

• Same-day documentation; CMS/regulatory compliance
