Job summary

CommuniCare Health seeks a Nurse Practitioner to provide high-quality primary care at E.A. Hawse Healthcare Center in Baker, WV. This part-time, hourly role requires diagnosing, treating, and coordinating care with the team to support residents and families.

The NP collaborates with PCPs and the care team in a value-based, patient-centered model, ensuring compliant documentation and adherence to regulations while delivering compassionate care.

Qualifications

• Master of Science in Nursing or Doctor of Nursing Practice
• National certification (AANP or ANCC)
• Active, unrestricted state license
• Active DEA license
• US work authorization
• 2+ years clinical experience in geriatric/adult/family practice
• Ability to work in a flexible environment and schedule
• Passionate about healthcare for seniors and low-income populations

Responsibilities

• Assess, diagnose, and manage chronic and acute conditions
• Provide health education and develop treatment plans
• Order and interpret labs and diagnostics and adjust plans
• Maintain EMR documentation
• Collaborate with PCPs and interdisciplinary teams
• Support value-based care initiatives
• Deliver excellent patient experiences to foster satisfaction
