Job summary

Personalized Health Partners (PHP) seeks a Nurse Practitioner to provide high-quality primary care to residents at E.A. Hawse Healthcare Center in Baker, WV.

The role is part-time, hourly, with three days per week, requiring collaboration with PCPs and interdisciplinary teams in a person-centered care model. Responsibilities include diagnosing and managing chronic and acute conditions, health education, treatment planning, and coordinating with telemedicine providers and family conferences.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP certificate program.
• Active, unrestricted state license.
• US work authorization.
• 2+ years clinical NP experience in geriatric, adult, or family practice.
• Active DEA license.

Responsibilities

• Assess, diagnose, and manage chronic and acute conditions.
• Provide health education to residents and families.
• Develop treatment plans and communicate with residents and other providers.
• Order and interpret labs and adjust treatment plans.
