Job summary

CommuniCare Family of Companies seeks a Nurse Practitioner (Adult and/or Gerontology) for a part-time, hourly position at Rainelle Healthcare Center in Rainelle, WV. The NP will deliver high-quality primary care in a high-touch, person-centered model across facilities and telehealth.

The role requires an MS/NP, national certification (AANP/ANCC), active state license, DEA license, and 2+ years of geriatric or adult clinical experience, with flexibility to work three days per week.

Qualifications

• MSN or DNP or NP certificate
• National certification (AANP/ANCC)
• Active, unrestricted state license
• Active DEA license
• US work authorization
• 2+ years geriatric/adult/family NP clinical experience

Responsibilities

• Assess, diagnose, and manage chronic and acute conditions in adults/geriatric patients.
• Provide health education and develop treatment plans.
• Order and interpret labs and diagnostics; adjust plans as needed.
• Maintain EMR documentation and coordinate with PCPs and care teams.
• Participate in family conferences and advance care planning; promote patient-centered care.
