Job summary

Center for Elders' Independence in California is seeking a Nurse Practitioner to join its interdisciplinary IDT under the PACE model, providing medical services to a frail elderly population at primary care clinics.

Reporting to the CMO and under PCP supervision, you will diagnose, treat, and manage care, collaborate with social workers, nurses, and therapists, and ensure documentation and adherence to protocols.

Qualifications

• Masters prepared Nurse Practitioner with CA BRN license and board certification.
• Minimum of one year of experience with frail or elderly clients.
• DEA certificate and Furnishing License required in California.
• Current CPR certification.
• Ability to work in an interdisciplinary care team.

Responsibilities

• Diagnose and treat acute health care problems; may order/perform diagnostic tests.
• Provide routine healthcare and conduct assessments.
• Consult with staff physician per NP protocol.
• Develop treatment plans and care plans with IDT.
• Counsel patients on drug regimens and side effects.
• Communicate care needs to outside consultants and caregivers.
• Document care in medical records and maintain policy compliance.
• Provide guidance to clinic nursing and support staff.
• Perform related duties as assigned.
