You'll participate in facility care team meetings, maintain open communication with families, ensure care plans align with members’ goals, and function autonomously while delivering high‑quality, patient‑centred care.

Qualifications

• MSN required as NP from a nationally accredited program.
• National board certification (AANP/ANCC/AACN) required.
• Experience in nursing home environment preferred.

Responsibilities

• Provide collaborative primary care to a panel of members.
• Develop and implement care plans for chronically ill residents.
• Communicate with families and care teams; coordinate with PCPs.
