Job summary

Longevity Health is seeking a Nurse Practitioner or Physician Assistant to manage a panel of nursing-home members with a focus on comprehensive assessments and individualized care plans. You will be supported by a dedicated clinical leadership team and collaborate to deliver high-quality, patient-centered care.

You will work autonomously in a nursing-home setting, coordinate with families and care teams, and ensure care aligns with members’ goals and needs. Licensure and DEA are required.

Qualifications

• 6+ months experience as a Nurse Practitioner or Physician Assistant.
• MSN as Nursing Practitioner from a nationally accredited program OR Master’s Degree from a nationally accredited program.
• Experience working in nursing home environments is strongly desired.
• Computer/Technology proficiency required.
• Must have an active national provider number (NPI) and an active Medicare provider number.
• DEA License required.

Responsibilities

• Provides collaborative primary care and services to a panel of members.
• Delivers ISNP Model of Care activities and develops advanced care planning.
• Facilitates and administers health plan clinical operations functions—prior authorization, Part A and B services, medication reviews, quality gap closure.
• Serves as a trusted advisor to members, loved ones and nursing home staff.
• Delivers effective and frequent communication with members’ PCPs, loved ones and care team members on care needs and goals.
• Establishes meaningful relationships and operates autonomously in the nursing home environment.
