Job summary

Longevity Health Plan is seeking a Nurse Practitioner or Physician Assistant to provide primary care for a panel of elderly residents in long-term care facilities. You’ll conduct comprehensive assessments, develop care plans, and work with an interdisciplinary team.

Join a dedicated clinical leadership team and contribute to reducing hospitalizations while coordinating with families and PCPs to align care with patient goals and needs.

Qualifications

• 6+ months experience as a Nurse Practitioner or Physician Assistant.
• MSN for NP or an equivalent master's degree from a nationally accredited program.
• Experience in nursing home environment is strongly desired.
• Computer/Technology proficiency required.
• Ability to engage with physicians, members, and families.

Responsibilities

• Provides collaborative primary care to a panel of members.
• Delivers ISNP Model of Care activities and advanced care planning.
• Facilitates clinical operations functions including prior authorization, Part A and B services, medication reviews and quality gap closure.
• Serves as a trusted advisor to members, families, and nursing home staff.
• Maintains frequent communication with members' families and care teams and aligns care with goals.
• Supports market goals and balances member caseloads.
• Promotes Longevity Health with stakeholders.
• Functions autonomously and maintains accountability for practice.
• Establishes meaningful relationships in the nursing home environment.
• Member-centric, high-quality care delivery.
• Performs other related duties as assigned.
