Job summary

Scarborough Health Network is seeking a Nurse Practitioner to lead geriatric care in the GAIN Clinic. The NP will collaborate with a multidisciplinary team to assess and manage acute and chronic conditions in adults and seniors, with emphasis on health promotion and safety.

The role includes outreach to seniors in the community and in LTC/retirement homes. Hours include days, nights and potential weekend work; liability coverage is required.

Qualifications

• Masters of Nursing required.
• Current registration in the Extended Class with the College of Nurses of Ontario or equivalent.
• Experience with geriatric patients preferred and active involvement in inter-professional teams.

Responsibilities

• Lead geriatric care within the GAIN Clinic, focusing on health promotion and function for frail seniors.
• Provide outreach consultations to LTC/retirement homes and community settings, including home visits as needed.
• Offer consultation liaison services for inpatients and ED patients with complex medical needs.
• Flexibility to work evenings and weekends; may include home visits; maintain liability coverage.
