Job summary

Ontario Shores Centre for Mental Health Sciences is seeking a Temporary (12 months) Full-Time Nurse Practitioner for the Geriatric Transitional Outreach (GTO) Clinic in Durham Community Outreach. The NP will collaborate with an interprofessional team to assess and treat older adults with complex mental health needs and dementia-related conditions.

Qualifications

• Graduate Degree (Master’s) preferred.
• Current Registration as Registered Nurse – Extended Class (RNEC).
• Certification in Mental Health Nursing preferred.
• Certification in Gerontological Nursing preferred.
• Certification in Canadian Academy of Geriatric Psychiatry (preferred).

Responsibilities

• Acts as Most Responsible Practitioner (MRP) for patients admitted to this outpatient service.
• Performs initial and ongoing psychiatric, medical and cognitive assessments.
• May prescribe, initiate, monitor and adjust medications within legislated scope of practice.
• Provides family support, education and counseling on diagnosis, treatment options and follow-up plans.
• Documents in accordance with professional standards of practice and Centre policies.
