Qualifications

• APRN or PA licensure required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations or Fee-for-Service experience.
• Electronic Health Records experience.
• Proficiency with Microsoft Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and create plans of care.
• Ensure compliance with local, state, and federal regulations for clinical services.
• Prescribe medications and order labs, diagnostics and consultations.
• Monitor patient progress and adjust treatment plans as needed.
