Job summary

Element Care in Lynn, MA, is seeking a Registered Nurse to join the PACE Interdisciplinary Team. You will participate in planning, implementing and evaluating care plans that meet the objectives, standards and policies of the PACE model of care to help frail elders remain living in the community safely.

You will provide direct patient care, coordinate medications, educate participants and caregivers, and collaborate with physicians and other professionals.

Qualifications

• Registered Nurse license in the Commonwealth of MA is required.
• Minimum of two years of experience with the geriatric population.
• Proficient in the use of Microsoft Office tools.
• Ability to travel locally on a frequent basis in a personal vehicle.
• Ability to recognize situations/problems early and implement effective solutions.

Responsibilities

• Provides direct care to participants and develops/revises care plans.
• Collects medical and social information to inform care decisions.
• Monitors care delivery to ensure quality standards.
• Coordinates with the interdisciplinary team to develop comprehensive care plans.
• Manages medications including stocking, ordering, and home delivery when needed.
• Educates participants and caregivers about care plans and medications.
• Communicates participant status to primary caregivers and the team.
• Adheres to safety standards, including fit testing for respirators.
