Job summary

Element Care in Massachusetts seeks a Personal Care Specialist to join the PACE Interdisciplinary Team under a licensed nurse. You will assist participants with personal care at the site and in the community, help with meals (including 1:1 feedings), and monitor vital signs.

Reporting changes to nursing staff, escorting to appointments, and maintaining clean spaces are part of the role. Strong communication and a compassionate approach are essential.

Qualifications

• High School diploma or equivalent preferred.
• HHA certification or equivalent.
• Minimum one (1) year of experience with the geriatric population.
• Ability to relate well to elders and anticipate their needs.
• Ability to perform vital signs, lifting of participants for transfer, and communication of participant changes.
• Effective communication, verbal and written.
• Consistently works in a positive and cooperative manner with team.

Responsibilities

• Assists participants with all aspects of personal care at the site and in the community.
• Assists with meals at the site, including 1:1 feedings, following diet and fluid restrictions.
• Assists Nursing with specimen collections, oxygen and vital sign monitoring.
• Reports medical changes to the primary nurse.
• Serves as an escort for medical appointments as needed.
• Restocks supplies and maintains cleanliness of rooms, bathrooms, kitchen and closets.
• Sanitizes tables, chairs and exam areas; adheres to policies and procedures.
• Performs other duties as required.
