Job summary

Britt Medical in Lynn, MA is seeking a BE/BC Geriatric Medicine Physician to join our PACE Interdisciplinary Team just north of Boston, delivering comprehensive, goal-focused care to high-risk older adults in the community.

This full-time role, 8am-4pm, no nights or weekends, involves home visits, telehealth when appropriate, and collaboration with the Interdisciplinary Team to create individualized care plans, including palliative and end-of-life care.

Qualifications

• Board eligible or certified in geriatrics (BE/BC)
• Experience with frail, low-income, diverse populations preferred
• Bilingual is desirable

Responsibilities

• Perform comprehensive physical examinations and assessments
• Evaluate participants for episodic visits and follow-ups
• Conduct nursing home visits per state requirements
• Utilize telehealth when appropriate
• Participate in the PACE hospital rounding system and on-call schedule
• Develop treatment plans with the Interdisciplinary Team
• Provide palliative and end-of-life care as needed
• Arrange and perform home visits as required
