Job summary

Trinity Health PACE in Massachusetts is seeking a Part-time Nurse Practitioner to deliver primary medical management in collaboration with PACE physicians. You will assess patients, develop care plans, and ensure comprehensive, patient-centered care.

The position includes discharge planning, treatment of acute illnesses, home visits, and participation in policy development and on-call rotations. Requires NP with prescriptive authority and BLS; CDS/DEA and ANCC/AANP board preferred.

Qualifications

• Master's degree in Nursing with 5–7 years of experience preferred in long-term care.
• Certification as Nurse Practitioner with prescriptive authority required.
• Current BLS certification required.
• CDS and DEA registration required.

Responsibilities

• Collaborate with PO physicians, assessing and implementing care plans.
• Participate in discharge planning and provide treatment for acute illness.
• Conduct home visits as needed.
• Assist in formulating clinical policies and procedures.
• Participate in provider on-call rotation.
