Job summary

Paycom - ATS is seeking a dedicated Nurse Practitioner to provide high‑quality primary and transitional care in Skilled Nursing Facilities, Assisted Living Facilities and Independent Living Facilities across multiple counties. The role requires daily travel and on‑site presence, with typical 4–6 hours per facility.

You will assess, diagnose, and manage conditions, develop care plans, prescribe per Georgia regulations, and coordinate with facility staff and families to improve outcomes and reduce

Qualifications

• Active and unrestricted Georgia APRN license.
• Board Eligible or Board Certified Nurse Practitioner.
• Experience in SNF/ALF/ILF or geriatrics preferred.
• Valid driver's license and reliable transportation.
• Malpractice insurance coverage eligible.

Responsibilities

• Perform comprehensive patient assessments in SNF/ALF/ILF and homes.
• Diagnose and manage acute and chronic conditions.
• Develop and implement individualized care plans.
• Prescribe medications per Georgia regulations and guidelines.
• Coordinate care with facility staff, specialists, and families.
• Review labs/imaging and follow up promptly.
• Respond to urgent clinical needs and document encounters within 48 hours.
• Participate in interdisciplinary rounds and meetings.
• Meet productivity and visit expectations.
• Travel between facilities and patient homes as required.
• Utilize EMR and mobile tools for documentation and communication.
