Qualifications

• Active, unrestricted NP license in Ohio.
• Board certification in family, adult, or geriatric care.
• DEA licensure.
• Experience in long-term care settings.
• Experience with an EMR system.

Responsabilités

• Provide primary and preventative care for long-term care residents in skilled nursing facilities.
• Manage residents with acute changes in condition.
• Advanced care planning and chronic care management.
• Prescribe medications, including controlled substances, per OH law.
• Order, review and follow up on treatments and diagnostics.
• Communicate with nursing home staff regularly about changes in condition.
• Collaborate with primary care physician.
