CareMed Health Solutions is seeking a Nurse Practitioner to provide primary and preventive care for residents in skilled nursing facilities in Ohio. You will manage acute changes, participate in advanced care planning, and collaborate with the interdisciplinary team to optimize patient outcomes.

You must hold an active Ohio NP license, board certification, and DEA licensure, with a background in geriatric care and experience in long-term care. EMR proficiency is required.

Qualificações

• Active, unrestricted NP license in Ohio.
• Board certification as NP.
• DEA licensure required.
• Clinical background in adult, family or geriatric care.
• Experience in long-term care setting preferred.
• Proficient with EMR systems.

Responsabilidades

• Provide primary and preventive care for residents in skilled nursing facilities.
• Manage residents with acute changes in condition.
• Advanced care planning and chronic care management.
• Prescribe medications per Ohio law.
• Order and review diagnostic tests and treatments.
• Coordinate with nursing home staff on changes in condition.
• Collaborate with primary care physicians.
