Job summary

CareMed Health Solutions in Ohio is seeking a Nurse Practitioner to deliver primary and preventive care to residents living in skilled nursing facilities, with a focus on improving quality of life for elderly patients. You will manage acute changes, perform advanced care planning, order tests, and collaborate with primary care physicians within an interdisciplinary team.

Requirements include an Ohio NP license, board certification, DEA licensure, and experience in geriatric or adult primary

Qualifications

Current, unrestricted NP license in Ohio.
Board Certification required.
DEA licensure required.
Clinical background in adult, family or geriatric specialties.
Experience in long-term care setting.
Proficient with EMR systems.
Ability to manage patients with complex, chronic conditions.
Work within an interdisciplinary care team.
Psych experience is a plus.

Responsibilities

Provide primary and preventative care for long-term care residents in skilled nursing facilities.
Manage residents with acute changes in condition.
Perform advanced care planning and chronic care management.
Prescribe medications in accordance with OH law, including controlled substances.
Order, review and follow up with treatments and diagnostic testing.
Communicate with nursing home staff about changes in condition and care plans.
Collaborate with primary care physicians as part of the care team.
