AlignMed Partners is seeking a full‑time Advanced Practice Provider at our Uptown Rehabilitation Center in Albuquerque, NM. The position supports nursing center residents with primary and urgent care, in collaboration with the Medical Director and center leadership.

The role welcomes new grads. Requires NP or PA credentials and state licensure; offers a base salary of $115,000–$125,000 plus a no‑cap incentive bonus, with comprehensive benefits and relocation support.

Qualifications

• Graduate from an accredited School of Nursing with RN license and NP certification.
• NP with Older Adult and/or Family NP concentration preferred; other concentrations may be considered.
• Physician Assistant must be ARC-PA accredited, PANCE passed, PA licensure and NCCPA certified.

Responsibilities

• Provide primary and urgent care to nursing center residents in collaboration with other providers.
• Collaborate with Center Leadership and Medical Director to meet center goals.
• Support AlignMed Partners mission with high‑quality care and everyday compassion.

Education

Master's Degree in Nursing (Older Adult/Family NP)

NP certification (MSN/DNP)
