Job summary

Personalized Health Partners (PHP) in Connersville, IN seeks a Nurse Practitioner for a part-time, hourly role at Caroleton Healthcare Center. First shift, never on-call, no weekends, with care for seniors across skilled nursing and related settings, including telehealth.

The PHP NP provides primary care, diagnoses and treats acute and chronic conditions, collaborates with PCPs and the care team, and emphasizes patient-centered planning and education.

Qualifications

• MSN or DNP or NP certificate with national certification (AANP/ANCC).
• Active, unrestricted state NP license and valid DEA registration.
• US work authorization and 2+ years of geriatric/adult/family NP experience.
• Ability to work in a flexible environment and travel to facilities if needed.

Responsibilities

• Assess, diagnose, and manage chronic and acute conditions.
• Provide health education to residents and families.
• Develop treatment plans and communicate them to providers and caregivers.
• Order and interpret labs and diagnostics; adjust plans accordingly.
• Maintain compliant EMR documentation.
• Collaborate with PCPs and interdisciplinary teams to manage care.
• Support value-based care initiatives and quality metrics.
• Ensure excellent customer experience and resident satisfaction.
