Job summary

TruHealth is seeking an advanced practice provider, either a nurse practitioner (NP) or physician assistant (PA) with geriatrics focus, to improve outcomes for long-term care residents. You’ll collaborate with a team of providers and case managers to deliver coordinated health care that enhances quality of life.

Our AP providers work primarily in nursing homes or assisted living communities enrolled in Medicare Advantage plans, exercising independent judgement within regulatory guidelines.

Qualifications

• FNP, AGNP, AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft applications, including Word, Outlook and Excel.

Responsibilities

• Assess patients' medical and mental health needs and plan proactive care.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order labs, procedures and consultations.
• Monitor treatment response and adjust plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
