Job summary

TruHealth is a healthcare provider focused on improving outcomes for residents in long-term care. We hire advanced practice providers (NP/PA) to deliver coordinated care within nursing homes and assisted living communities enrolled in Medicare Advantage plans.

You will treat acute and chronic conditions, consult with supervising physicians, and work with facility partners to implement a national model of care.

Qualifications

• APRN or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft Word/Outlook/Excel.

Responsibilities

• Assess patients’ medical and mental health needs and provide a plan of care that proactively manages conditions.
• Ensure compliance with local, state, and federal regulations related to clinical services you provide.
• Prescribe medications and order labs, procedures and consultations.
• Monitor patient’s treatment response and adjust plans accordingly.
• Collaborate with RN case managers as part of an integrated care team.
