Job summary

TruHealth is seeking an advanced practice provider, either a nurse practitioner (NP) or physician assistant (PA), with experience in primary care or internal medicine and a geriatrics focus. The role centers on delivering coordinated care to residents in long-term care settings, collaborating with a team to improve quality of life and reduce hospital visits.

APRN/PA license is required with a Master’s degree; at least 3 years in geriatric clinical nursing or rehab is preferred.

Qualifications

• APRN, AGNP, AHACNP or PA license required.
• Masters or better from an accredited APRN/PA program.
• 3 years of clinical nursing or rehab in geriatric populations.
• Experience with Electronic Health Records and MS Office.

Responsibilities

• Assess patients' medical and mental health needs with a proactive plan of care.
• Ensure compliance with local, state, and federal regulations for clinical services.
• Prescribe medications and order labs, diagnostics, and consultations.
• Monitor treatment response and adjust plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
