Job summary

TruHealth, a division of American Health Partners, is seeking an advanced practice provider (NP or PA) to deliver primary care in long-term care settings.

Work collaboratively with a team of clinicians to assess residents, manage chronic conditions, and implement care plans while ensuring compliance with regulatory standards. A $5,000 sign-on bonus is offered and opportunities for career growth are available.

Licensure in APRN or PA and experience in geriatric populations are preferred.

Qualifications

• APRN or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations and/or Fee for Service experience.
• Electronic Health Records experience.
• Working knowledge of Microsoft applications, including Word, Outlook and Excel.

Responsibilities

• Assessing patients’ medical and mental health needs and providing a plan of care that proactively manages their conditions and addresses barriers to care.
• Ensuring compliance with local, state, and federal agencies related to clinical services you provide.
• Prescribing medications and ordering lab work, diagnostic procedures and consultations.
• Monitoring patients’ compliance and response to their treatment and modifying those plans.
