Job summary

Longevity Health is seeking a Nurse Practitioner or Physician Assistant to manage a panel of long-term care members. You will conduct comprehensive assessments, develop individualized care plans, and work with a multidisciplinary team to align care with each member’s goals.

You’ll collaborate with families, participate in facility care meetings, and ensure timely communication. Candidates should hold an NP or PA license, with experience in nursing homes and strong clinical skills.

Qualifications

• 6+ months experience as NP or PA
• Master of Science in Nursing as NP or equivalent master’s degree
• Experience in nursing home environment is strongly desired
• Computer/Technology proficiency
• Effective communication with physicians, members and families
• Self-directed but works well in a team
• Strong clinical skills—assessment, diagnosis, and treatment of chronically ill
• Demonstrated organizational and prioritization skills

Responsibilities

• Provides collaborative primary care to a panel of members
• Delivers ISNP Model of Care activities and advanced care planning
• Handles care coordination: prior authorization, Parts A&B, medication reviews, quality gaps
• Acts as trusted advisor to members, families and staff
• Communicates with PCPs and care teams about goals and changes in care
• Supports market goals and balances member caseloads
• Promotes Longevity Health with stakeholders
• Functions autonomously and is accountable for practice
• Builds meaningful relationships in nursing home environments
• Delivers high-quality, member-centric care
• Performs other duties as assigned
