Job summary

AlignMed Partners is seeking a full-time Advanced Practice Provider for our Heritage Center in Huntington, WV. The role can be filled by a nurse practitioner or physician assistant, with new grad NPs welcome. The base salary range is USD 100,000–USD 116,480 per year, plus an incentive bonus program.

You will deliver primary and urgent care to nursing center residents, collaborating with leadership and the Medical Director to ensure high-quality care and alignment with organizational goals.

Qualifications

RN licensure and NP certification (Older Adult and/or Family NP) with a Master's in Nursing.
PA must have ARC-PA-accredited program and PANCE, with state licensure and NCCPA certification.
Minimum two years of experience with adult or geriatric populations.

Responsibilities

Provide primary and urgent care to nursing center residents with other providers.
Collaborate with Center Leadership and Medical Director on goals.
Work on-site with leadership to achieve AlignMed Partners mission.
