Job summary

HarmonyCares is seeking a Nurse Practitioner to provide geriatric chronic condition management in home health across Kent, Federal Way, Auburn and Renton. This three-day-per-week role emphasizes value-based, patient-focused care for high-acuity patients in patients' homes.

Qualified candidates hold WA NP licensure, DEA eligibility, CPR/BLS certification, and board certification. A valid driver’s license and clean driving record are required, with comprehensive benefits and opportunities for CME

Qualifications

• Licensed NP in the state where HarmonyCares Medical Group office is located (WA).
• Federal DEA License/eligibility.
• Geriatric training/experience preferred.
• Active CPR/BLS Certification.
• Board Certification.
• Must maintain a valid driver’s license and good driving record.
