Job summary

CareMed Health Solutions is seeking a Nurse Practitioner to provide primary and preventive care to residents in long-term care facilities in Rochester, NH. The NP will manage acute changes, engage in advanced care planning, and coordinate with primary care physicians to ensure comprehensive geriatric care.

Responsibilities include prescribing medications per NH law, ordering and interpreting tests, and collaborating with facility staff to optimize patient outcomes.

Qualifications

• Current, unrestricted NP license in New Hampshire.
• Board Certification in a relevant NP specialty.
• DEA licensure.
• Clinical background in adult, family or geriatric care.
• Experience working in a long-term care setting.
• Ability to use an EMR.

Responsibilities

• Provide primary and preventative care for residents in long-term care facilities.
• Manage residents with acute changes in condition.
• Engage in advanced care planning and chronic care management.
• Prescribe medications, including controlled substances per NH law.
• Order, review and follow up on treatments and tests.
• Communicate with nursing home staff regarding changes in condition and care plans.
• Collaborate with primary care physicians.
