Job summary

CareMed Health Solutions seeks a Nurse Practitioner to deliver primary and preventive care to residents in long-term care facilities. You will work within an interdisciplinary team to coordinate patient care and respond to acute changes in condition.

Responsibilities include advanced care planning, chronic care management, prescribing medications per Colorado law, ordering tests, and collaborating with facility staff and the resident's primary physician.

Qualifications

• Licensed NP in Colorado with board certification and DEA license.
• Experience in long-term care and geriatrics.
• Proficient with EMR and interdisciplinary team coordination.

Responsibilities

• Provide primary and preventive care in skilled nursing facilities.
• Manage residents with acute changes in condition.
• Advanced care planning and chronic care management.
• Prescribe medications per Colorado law.
• Order and follow up on treatments and diagnostics.
• Coordinate with nursing home staff and primary physician.
