CareMed Health Solutions seeks a Nurse Practitioner to provide primary and preventive care to residents in skilled nursing facilities across New Hampshire.

You will manage acute changes, implement care plans, order tests, collaborate with physicians, and work within an interdisciplinary team to deliver compassionate geriatric care in a supportive facility setting. Experience with EMR and chronic care management is preferred.

Qualifications

• Current, unrestricted NP license in the state of New Hampshire.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team to coordinate patient care.
• Psych experience a plus

Responsabilités

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
• Manage Residents with acute change in condition.
• Advanced care planning and chronic care management.
• Prescribe appropriate medications, including controlled substances in accordance with NH Law.
• Order, review and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition and/or the care plan.
• Collaborate with primary care physician.
