Job summary

TruHealth, a division of American Health Partners, is seeking an advanced practice provider (NP or PA) with geriatrics experience to deliver primary care within nursing homes or long-term care facilities. Earn a competitive compensation package with a $5,000 sign-on bonus and collaborative team support.

You will assess medical and mental health needs, prescribe medications, order labs, and coordinate care while adhering to federal and state regulations to improve resident outcomes.

Qualifications

• APRN or PA license is required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations and/or Fee for Service experience.
• Electronic Health Records experience.
• Working knowledge of Microsoft Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and provide a plan of care.
• Ensure compliance with local, state, and federal agencies related to clinical services.
• Prescribe medications and order lab work, diagnostic procedures and consultations.
• Monitor patients’ compliance and response to treatment and modify plans as needed.
