Job summary

TruHealth, part of American Health Partners, seeks an advanced practice provider (NP or PA) with geriatric focus for primary care in long-term care settings. You’ll collaborate with physicians, case managers, and facility leaders to deliver proactive, personalized care and reduce hospital visits.

Responsibilities include assessing needs, prescribing meds, ordering tests, and ensuring regulatory compliance while using EHRs and MS Office tools. Strong geriatrics experience preferred.

Qualifications

• Masters or higher in nursing or related APRN program.
• APRN or PA program accreditation required.
• 3 years of clinical nursing or rehab in geriatric populations.
• EHR experience and basic MS Office skills.

Responsibilities

• Assess medical and mental health needs and create proactive care plans.
• Ensure compliance with local, state and federal healthcare regulations.
• Prescribe medications and order labs, diagnostics and consults.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
