Qualifications

• Colorado licensure as an LCSW, LMFT, or LPC required.
• National Provider Identifier (NPI) is required.
• Professional Liability Insurance must be provided.
• Credentialing with Medicaid/Medicare is a plus.

Responsabilités

• Complete diagnostic evaluations and treatment planning.
• Collaborate with multidisciplinary teams for patient care.
• Document in EMR (Athena I) timely.
• Ensure compliance with agency policies.
