Qualifications

• Montana licensure as an LCSW, LMFT, or LPC required.
• NPI (National Provider Identifier) is necessary.
• Professional Liability Insurance is mandatory.
• Credentialing with Medicaid/Medicare is a plus.
• TB Test is required.
• CPR/AED/First Aid Certification is required.

Responsibilities

• Completes diagnostic evaluations and treatment planning.
• Collaborates with other healthcare professionals for patient care.
• Documents patient information in EMR on time.
• Ensures compliance with agency policies.
