A mental health service provider in Colorado is seeking full-time independently licensed therapists to join their therapy team. Qualifications

• Colorado licensure as an LCSW, LMFT, or LPC.
• NPI (National Provider Identifier).
• Professional Liability Insurance.
• Some facilities may require up-to-date vaccinations or appropriate exemptions.
• TB Test.
• CPR/AED/First Aid Certification.

Responsabilités

• Completes diagnostic evaluations and treatment planning in Skilled Nursing Facilities.
• Completes documentation remotely in EMR (Athena I).
• Ensures compliance with agency policies.
