Qualifications

• Must have Colorado licensure as an LCSW, LMFT, or LPC.
• Must have NPI (National Provider Identifier).
• Credentialed with Medicaid/Medicare is a plus.
• TB Test and CPR/AED/First Aid Certification are required.

Responsibilities

• Complete diagnostic evaluations and treatment planning.
• Collaborate with psychiatrists and facility staff for patient care.
• Complete documentation remotely in EMR (Athena I).
• Ensure compliance with agency policies and procedures.
