Job summary

Behavioral Health Solutions is seeking Independently Licensed Therapists (LICSW, LMFT, LPC) to join our SNF Therapy team across Colorado. The role focuses on geriatric behavioral health in Skilled Nursing and Long Term Care facilities, with a strong emphasis on coordinated, whole-person care.

Qualifications

• Master's degree in Social Work, Counseling, or similar field and Colorado licensure as LCSW/LMFT/LPC.
• NPI (National Provider Identifier) is preferred.
• Credentialed with Medicaid/Medicare is a plus.
• TB test and CPR/AED/First Aid certification may be required.
• Vaccinations may be required at some facilities.

Responsibilities

• Completes diagnostic evaluations, psychotherapy, treatment planning, and consultations in‑person at SNF/LTC facilities.
• Collaborates with psychiatrists, psychologists, care coordinators, and staff for holistic care.
• Completes documentation in a timely fashion in the EMR (Athena I).
• Complies with agency policies and procedures.
