Job summary

Vitae Health Systems is seeking a qualified Psychotherapist to provide mental health services to residents in a skilled nursing facility. You will assess, diagnose, and treat emotional and behavioral conditions, supporting residents’ overall well-being and adjustment to medical and life changes.

Qualifications

• Master’s degree in Psychology, Counseling, Social Work, or related field.
• Active and unrestricted license in respective state (e.g., LCSW, LCPC, LMFT, or Licensed Psychologist).
• 2 years of clinical experience preferred, ideally in geriatric, long-term care, or medical settings.
• Proficiency in evidence-based therapeutic approaches (e.g., CBT, trauma-informed care, grief counseling).
• Ability to work collaboratively within an interdisciplinary team.
• Excellent communication and interpersonal skills with residents, families, and staff.

Responsibilities

• Conduct psychosocial assessments for residents upon admission and as needed.
• Develop, implement, and monitor individualized treatment plans.
• Provide individual, group, and family therapy sessions.
• Address depression, anxiety, grief, cognitive decline, and adjustment to illness or disability.
• Support residents with chronic illness, pain, loss of independence, and end-of-life concerns.
• Collaborate with the Vitae clinical team, attending physicians, facility nursing staff, social workers, and rehabilitation teams to coordinate care.
• Participate in care plan meetings and interdisciplinary discussions.
• Maintain accurate clinical documentation in compliance with regulations.
• Provide crisis intervention and behavioral management strategies when needed.
• Educate staff and families on mental health conditions and effective communication strategies.
• Ensure HIPAA compliance and ethical standards of practice.
