Vitae Health Systems, a multi-specialty healthcare organization headquartered in Chicago, delivers medical and behavioral health services in skilled nursing facilities across the Midwest and East Coast. We are seeking a qualified Psychotherapist to assess, diagnose, and treat emotional and behavioral conditions, supporting residents’ well-being and adaptation to medical life changes.

This role offers a base salary range and bonus potential.

Qualificações

• Master’s degree in Psychology, Counseling, Social Work, or related field.
• Active and unrestricted license in the respective state (LCSW, LCPC, LMFT, or Licensed Psychologist).
• 2+ years of clinical experience preferred in geriatric, long-term care, or medical settings.
• Knowledge of SNF mental health needs and evidence-based approaches.
• Proficiency with electronic health records (EHR) preferred.

Responsabilidades

• Conduct comprehensive psychosocial assessments for residents.
• Develop, implement, and monitor individualized treatment plans.
• Provide individual, group, and family therapy sessions.
• Address depression, anxiety, dementia, and adjustment to illness.
• Collaborate with physicians, nurses, social workers, and rehab teams.
• Participate in care plan meetings and interdisciplinary discussions.
• Maintain accurate, timely clinical documentation per regulations.
• Provide crisis intervention and behavioral management as needed.
• Educate staff and families on mental health and communication.
• Ensure HIPAA compliance and ethical practice.
