Vitae Health Systems is seeking a qualified Psychotherapist to provide mental health services to residents in a skilled nursing facility. The role involves assessing, diagnosing, and treating emotional and behavioral conditions to support residents’ well-being during medical and life changes.

Responsibilities include psychosocial assessments, individualized treatment planning, and delivering therapy across individual, group, and family formats.

Qualifications

• Master’s degree in Psychology, Counseling, Social Work, or related field is required.
• Active license in relevant psychology/mental health field with state-specific authorization is required.
• Experience delivering mental health services in clinical settings is expected.

Responsabilités

• Conduct comprehensive psychosocial assessments for residents upon admission and as needed.
• Develop, implement, and monitor individualized treatment plans.
• Provide individual, group, and/or family therapy sessions.
• Address depression, anxiety, grief, cognitive decline, and adjustment to illness or disability.
• Collaborate with the interdisciplinary care team to coordinate resident care.
• Maintain accurate documentation in compliance with HIPAA, facility, state, and federal regulations.
