Job summary

Vitae Health Systems is seeking a qualified Psychotherapist to deliver mental health services to residents in a skilled nursing facility. You will assess, diagnose, and treat emotional and behavioral conditions, helping residents adjust to medical and life changes.

Responsibilities include psychosocial assessments, treatment planning, individual/group therapy, and collaboration with the care team to coordinate comprehensive care in a respected, interdisciplinary environment.

Qualifications

• Master’s degree in Psychology, Counseling, Social Work, or related field.
• Active and unrestricted clinical license in the respective state (LCSW, LPC, LMFT, or Psychologist).
• 2 years of clinical experience preferred, ideally in geriatric, long-term care, or medical settings.
• Strong assessment, diagnostic, and treatment planning skills.
• Familiarity with EHR systems and HIPAA compliance.
• Experience with evidence-based therapies (e.g., CBT, trauma-informed care).

Responsibilities

• Provide psychosocial assessments for residents upon admission and as needed.
• Develop, implement, and monitor individualized treatment plans.
• Offer individual, group, and family therapy sessions.
• Address depression, anxiety, grief, dementia, and adjustment to illness.
• Collaborate with interdisciplinary team to coordinate care and participate in care plan meetings.
• Maintain timely clinical documentation in compliance with regulations.
• Provide crisis intervention and behavioral management when needed.
• Educate staff and families on mental health conditions and effective communication.
• Ensure HIPAA compliance and ethical practice.
